What is Your Decisions Matter?
SAGE Eldercare was awarded a grant from Atlantic Health System’s Overlook and Chilton Medical Centers’ Community Advisory Boards to develop a new community outreach program.
Your Decisions Matter, was launched in 2019 to tackle the often uncomfortable and emotionally charged topic of end-of-life care and decision making. The goal is to help remove the taboo from the topic so loved ones can openly discuss with their family what they want at the end of their lives, ideally before a medical crisis occurs. Normalizing this difficult topic will lead to peace of mind knowing that wishes will be followed, particularly at times when a patient is not able to advocate for themselves.
A recent survey found that 87% of people believe it is important to put their own end-of-life preferences in writing, but only 23% have actually done so.

Since COVID-19 began, we are hosting webinars and events via Zoom.
